Friends and Family Test
The Friends and Family Test is a quick way to tell us what you think of the surgery. Every NHS service uses it. Your answer is anonymous, and you can give one after any visit or phone call.
What we ask
The test asks one question: Overall, how was your experience of our service?
You choose one answer:
- very good
- good
- neither good nor poor
- poor
- very poor
- don't know
You can then tell us why, and what we could do better. This is optional.
How to give your answer
After an appointment we may send you a text message with a link to our short online form. You can also use the form at any time.
You can also:
- scan the QR code on the poster at reception
- ask our reception team for a paper form
You do not need to give your name.
You can leave a review of the surgery on the NHS website too.

This form is for feedback only
Nobody reads the Friends and Family Test for medical or admin requests, and we cannot reply to it, because it is anonymous. Do not use it to ask for an appointment, a prescription, a sick note, test results or anything else you need from us. Call us on 01708 552 072 or ask for an appointment instead.
If something has gone wrong
The Friends and Family Test is not the way to make a complaint. If you want us to look into a problem, contact us.

What we do with your answers
We look at every answer. We share them with our team each month and use them to decide what to improve. We also send the numbers to NHS England.
We keep your answer for 2 years. We never send your comments to NHS England, only the number of answers of each kind.
