Get a sick (fit) note
A fit note is the official name for a sick note. It says you are not fit for work, or that you may be fit for some work with changes.
If you have been off sick for 7 days or less
You do not need a fit note. Your employer can ask you to confirm you were ill. You do this by filling in a form yourself when you go back to work.
This is called self-certification. Get the form from your employer, or use the government's form.

If you have been off sick for more than 7 days
You need a fit note. The 7 days include weekends and days you do not usually work.
How to ask for a fit note
The quickest way is to send us a request online. Tell us what is wrong, when your illness started, and the dates you need the note to cover.

You can also call us on 01708 552 072, or ask at the front desk.
We will look at the information you give us. Then we will decide whether you need to speak to a doctor, nurse or other healthcare professional first. Please do not ask for an urgent appointment today just for a fit note.
We usually issue fit notes on Thursdays.
How you get your fit note
We send your fit note to you by email. Make sure we have your current email address.
A doctor, nurse, pharmacist, physiotherapist or occupational therapist can give you a fit note.
If you were in hospital
The hospital gives you a fit note for the whole time you need to recover. Ask the ward staff, even after you have gone home. You do not need to ask us.
Fit notes for time you have already had off
A fit note can cover time you were already off sick. Tell us the dates when you ask.
If your child is off school
Schools should not ask for a letter from a GP. A note from a parent or carer is enough.
Private sick notes and letters
Fit notes are free when you have been ill for more than 7 days. If your employer wants a note for a shorter illness, or extra information, this is private work.
There is a charge for private notes and letters. Fees vary. Ask at reception and we will tell you the cost before any work starts.
