Change your contact details
Tell us as soon as you can if you change your:
- address
- phone number
- email address
- name
We need your details to be up to date so we can contact you about appointments, results and reminders.
How to change your details
At the front desk
Come to the surgery and tell reception. We are open Monday to Friday, 8am to 6:30pm.
This is the only way to change your details. You cannot change your phone number, email address or address in the NHS App.
There is no form on this website. We do not take changes by email, by phone or through our online request form.
If you have changed your name
Bring the document with your new name on it, such as a marriage certificate or deed poll. A copy is fine. Show it at the front desk.
We cannot accept name-change documents through our online request form.
If you move house
Check that your new address is in our practice area. See our practice area map
If your new address is outside our area, you will need to join a surgery near your new home. Find out how to join a GP surgery (NHS website) (opens in a new tab)
Tell us if other members of your family who are patients here have moved too.
Update your pharmacy
If you want your prescriptions sent to a different pharmacy, you can change your chosen pharmacy. Find out how to change your pharmacy
If you are a carer
Tell us if you look after someone, or if someone looks after you. We will note it on your record so we can support you.
Text message reminders
When you give us a mobile number, we can send you text reminders about appointments. Tell reception if you do not want them.
