Join the surgery

Anyone who lives in our practice area can join the surgery. You do not need ID, proof of address, an NHS number or proof of your immigration status.

Check you live in our area

We can only take patients who live in our practice area. Check your address on the map before you apply.

If you live outside our area, you will need to join a surgery near your home. Find out how to join a GP surgery (NHS website) (opens in a new tab)

Join online

The quickest way to join is the NHS online service called Register with a GP surgery. It is run by the NHS, so your details are safe.

Fill in the form for each person who wants to join, including children.

We will set up your record within 10 to 14 working days.

Join on paper or with help

You can also fill in a paper form. Ask for one at the front desk at 1 Harlow Road, Rainham, RM13 7UP. We are open Monday to Friday, 8am to 6:30pm.

If you need help to fill in the form, our reception team will help you. Tell us if you need an interpreter, or the form in large print, easy read or another format.

If you do not know your NHS number

You do not need your NHS number to join. If you want to find it, it is on any NHS letter, or you can look it up online.

If you are visiting the area

If you are staying in our area for less than 3 months, you can join as a temporary patient. Ask at the front desk for the temporary patient form.

If you are not our patient and need urgent treatment, we can treat you for up to 14 days. After 14 days you need to join as a temporary or permanent patient.

If you are a student

If you are studying in our area, you can join the surgery for the time you live here. You can only be a patient at 1 surgery at a time. When you go home for the holidays, you can see a GP there as a temporary patient.

If you are a carer

Tell us when you join if you look after someone, or if someone looks after you. We will add a carer flag to your record so we can support you.

As a carer you can have:

  • longer or more flexible appointments
  • a free flu jab every year

Havering Carers Hub gives free support and advice to carers in Havering. Call 01708 961111 or email info@haveringcarershub.org.uk.

Your named GP

Every patient has a named GP. This includes children. Your named GP is responsible for making sure you get the care you need from us.

You can still see any doctor or nurse at the surgery. You do not have to see your named GP.

Your named GP is Dr Noreen Tariq. We will confirm this if you ask at reception or call us.

If you would prefer a different named GP, tell us. We will do our best to arrange it. You can also tell us if you do not want a named GP.

If you are aged 75 or over

If you are 75 or over, your named GP also:

  • takes the lead in making sure the care you need is planned and delivered
  • looks out for your physical and mental health needs
  • makes sure you get a health check if you ask for one
  • works with other health and care professionals involved in your care

After you join

We will send you a welcome email once your record is set up. If you need an appointment, call reception to book.

Once you have joined, you can use the NHS App to order repeat prescriptions, see your test results and see your record.

If you need help before your record is ready, call us on 01708 552 072. If it's a life-threatening emergency, call 999.
