Access your health record

Your health record holds your medicines, allergies, test results, letters and notes from your appointments. This page explains how to see it, how to get a copy, and how it is shared.

See your record in the NHS App

If you are aged 16 or over, you can see your GP health record online. Use the NHS App or the NHS website.

You can see your medicines, allergies, vaccinations, health conditions and test results. You can also see documents such as referral letters and letters from the hospital.

The NHS App shows entries from the date we switched on online record access, in late 2023. If you need older results or notes, ask us to make them available.

You may see a test result before a doctor has spoken to you about it. If something worries you, ask us. Do not worry alone.

Ask for a copy of your record

You have the right to a copy of your record. This is called a subject access request. There is no charge for a copy of your own record.

To ask for a copy of part of your record, fill in the form at reception and bring photo ID. We can print your copy or email it to you.

To ask for your full record, write to the Practice Manager at Harlow Road Surgery, 1 Harlow Road, Rainham, RM13 7UP. We will reply within 1 month.

Your Summary Care Record

Your Summary Care Record is a short summary of your GP record. It shows your medicines, your allergies and any bad reactions you have had to medicines. Staff who treat you elsewhere, such as at a hospital or an out-of-hours service, can see it.

Everyone registered with a GP in England has one, unless they choose not to. You can ask us to add more information, such as your health conditions. You can also ask us to opt you out.

Your shared care record in north east London

We share your record through the London Care Record so hospitals, out-of-hours GPs and community teams can see it when treating you. Tell reception if you want to opt out.

Only staff caring for you can see it, and every look is recorded. This means you do not have to repeat your history.

Choose how your data is used for research and planning

The NHS also uses health information to plan services and for research. You can opt out of this. It is called the national data opt-out, and it does not affect your own care.

You can make your choice if you are aged 13 or over. Do it online, or call 0300 303 5678, Monday to Friday, 9am to 5pm. Parents can choose for children under 13.

If something in your record is wrong

Tell us. We will correct mistakes about facts, such as your address or date of birth. We cannot change a doctor's opinion, but we can add a note to say you disagree.
